Your ribs attach to the spine at two small joints per rib — the costovertebral joints. When one of these becomes stuck or irritated, the pain is sharp, well-localized, and often catches with deep breathing, coughing, or twisting.
What it feels like
- Sharp, well-defined pain in the mid-back, off to one side of the spine
- Sometimes wrapping around the ribcage to the front or side
- Worse with deep breathing, sneezing, coughing, or laughing
- Worse with certain rotations or reaching movements
- Localized tenderness right over the affected joint
Common triggers
- Sleeping in an awkward position
- Sudden twisting movements (reaching into the back seat, an unexpected sneeze)
- Prolonged sustained postures at work
- Coughing episodes during respiratory illness
- Direct trauma to the ribs or upper back
Ruling out more serious causes
Sharp chest or upper back pain always deserves quick screening for cardiac, pulmonary, or serious spinal causes. Once cleared, mechanical rib pain is one of the most treatable musculoskeletal conditions.
How we treat it
Rib mobilization or manipulation — often produces immediate dramatic relief. When the rib re-articulates properly, breathing suddenly feels normal.
Soft tissue work for the intercostal muscles and thoracic paraspinals.
Breathing mechanics work — restoring rib motion during respiration.
Postural correction — the pattern often recurs in people with poor thoracic mobility.
