You know the feeling. You do a workout, sleep fine, wake up the next morning feeling maybe a little tight. Then day two hits — your legs are on fire, walking down stairs is a negotiation, sitting on a toilet requires strategy. That's delayed onset muscle soreness, or DOMS, and it's one of the most misunderstood phenomena in fitness.
What DOMS actually is (and isn't)
DOMS is the muscle soreness that develops 12-48 hours after unaccustomed exercise, peaks around 24-72 hours, and typically resolves within 3-5 days. It's driven by eccentric muscle contractions — the lengthening phase of an exercise, like the lowering portion of a squat or the deceleration during running downhill.
Here's what DOMS is not:
- It's not lactic acid buildup. This myth refuses to die but has been thoroughly debunked. Lactate is cleared from muscle within about an hour of exercise — it has nothing to do with the soreness you feel 48 hours later.
- It's not a reliable indicator of muscle growth. You can build significant strength and muscle without ever being sore, and being extremely sore doesn't mean you had a productive workout.
- It's not a sign you worked hard enough. The "no pain, no gain" mentality confuses acute exercise discomfort with training effectiveness. Chronic soreness often means you're recovering poorly, not training well.
What actually causes it
DOMS is caused by microscopic damage to muscle fibres and connective tissue, particularly during eccentric contractions. The subsequent inflammatory response — necessary for the muscle to rebuild stronger — is what produces the classic delayed soreness.
You experience the most DOMS when:
- You do a movement you're not accustomed to
- The exercise has a large eccentric component (heavy lowering, downhill running, negatives)
- The volume or intensity is significantly higher than your usual training
- You've had a training break and are returning
The good news is that DOMS produces the "repeated bout effect" — the same workout done a second or third time causes progressively less soreness. Your muscles adapt quickly.
What actually helps recovery
Here's where the research is genuinely interesting. Many popular recovery methods have far less evidence behind them than their popularity suggests. Let me walk through what the literature actually shows.
Sleep — the highest-impact recovery tool
Nothing else comes close. 7-9 hours of quality sleep is the single most effective recovery intervention. Sleep is when growth hormone peaks, muscle protein synthesis is optimized, and inflammation resolves. If your recovery is poor, sleep is where to invest first.
Protein and total nutrition
Adequate protein (roughly 1.6-2.2 g/kg body weight for active people) supports the muscle rebuilding process. Timing matters less than total daily intake. Fuelling adequately overall — not being in a chronic caloric deficit — makes a substantial difference.
Active recovery
Light movement — walking, easy cycling, swimming — the day after a hard workout consistently outperforms complete rest for reducing soreness. It increases blood flow, moves metabolic waste, and improves how the muscle feels without adding damage.
Massage and soft tissue work
The evidence for massage improving DOMS is actually pretty strong. Sports massage, deep tissue work, and instrument-assisted soft tissue mobilization (Graston) consistently reduce perceived soreness. The mechanism is likely a combination of improved circulation, reduced muscle tension, and nervous system modulation.
Hydration and electrolytes
Dehydration amplifies the perception of soreness and slows recovery. Keeping hydrated matters, but you don't need a specific "sports drink" for most training — water and normal meals cover it.
What helps less than people think
Static stretching
Stretching before or after training has consistently failed to reduce DOMS in controlled studies. It's not harmful, but don't count on it as a recovery strategy.
Ice baths and cold exposure
The evidence here is mixed and complicated. Cold water immersion can reduce acute soreness perception, but it may also blunt the muscle adaptation response — meaning less soreness but potentially less strength or muscle gain over time. For competitive athletes needing to perform back-to-back, they have a role. For most people trying to get stronger, they may be counterproductive.
Foam rolling
Foam rolling produces modest short-term reductions in soreness and stiffness. It's not magic, and it doesn't "release fascia" the way people describe. But it feels good and has no downside — use it if you enjoy it.
Anti-inflammatory medications (NSAIDs)
Ibuprofen and similar drugs suppress the inflammation that drives DOMS, but they also blunt the very adaptations you're training for. Repeated NSAID use in training athletes appears to reduce strength and muscle gains. Use them for injury when needed, not for post-workout soreness management.
When soreness is more than DOMS
Not all post-exercise pain is normal DOMS. Watch for these warning signs:
- Sharp, localized pain that doesn't fit the pattern of exercised muscles
- Swelling, bruising, or visible deformity
- Pain that gets worse rather than better over days
- Dark, tea-coloured urine after very heavy exercise (potential sign of rhabdomyolysis — get medical attention immediately)
- Pain that limits normal daily function for more than 5-7 days
These aren't DOMS — they're injuries that need proper assessment.
How I approach recovery in clinic
Many of my patients are athletes or gym-goers whose recovery has been suboptimal. My approach:
- Soft tissue work to reduce accumulated tension and speed recovery — Graston, ART, and cupping all have their place
- Chiropractic care to keep joints moving properly and reduce compensatory patterns
- Acupuncture for athletes with heavy training loads — particularly effective for chronic muscular tightness and sleep quality
- Programming and habit review — often the recovery issue is a sleep, nutrition, or training volume problem, not a treatment issue
The takeaway
DOMS is a normal part of pushing your body into new territory. It's not a badge of honour, it's not a reliable measure of workout quality, and it doesn't require aggressive intervention. The most powerful recovery tools are sleep, food, gentle movement, and time. Fancy modalities can help but shouldn't replace the fundamentals.

Often the answer isn't more recovery tools — it's addressing a specific compensation, mobility issue, or programming problem that's stealing your recovery capacity.
